The building dates to the 1940s and has been reworked by ACPV Architects Antonio Citterio Patricia Viel, who used large blocks of colour and imperial marble to link the interiors to the city's Roman heritage. The hotel holds 106 Rooms and Suites, arranged across categories that include Superior Rooms, Deluxe Rooms, Premium Rooms, Junior Suites, Superior Suites, Deluxe Suites, the Serpenti Suite, Premium Suites and the Bvlgari Suite, the last of which looks out over the Mausoleum of Augustus.

Dining runs from Il Ristorante, on the fifth floor under Chef Niko Romito, with mahogany walls, Bvlgari artwork and seating for up to 54 guests overlooking the Mausoleum, to Il Caffè on the ground floor beneath the porticos of the piazza. The Bvlgari Bar, open from 5pm to 1am, has a black marble counter finished with 110 Murano glass twirls hand blown with gold dust by Barovier&Toso, plus an outdoor bar and two terraces, one onto Via di Ripetta and one onto the piazza itself, with room for 108 guests inside and 108 out. The Bvlgari Lounge sits by the entrance foyer and serves everything from afternoon tea to aperitivo, while La Terrazza, the Champagne Bar and the Pizza Bar by Pier Daniele Seu round out the hotel's drinking and dining spaces. Italy's first Bvlgari Dolci chocolatier and patisserie, along with Il Gelato, complete the food offer beyond the restaurants.

The Bvlgari Spa covers 1500 square metres in a design that recalls ancient Roman baths, with marble columns rising from the water and coloured glass filtering the light. It holds eight treatment rooms, a Spa Suite with a green onyx bath and a 20-metre indoor pool set with mosaics by Bisazza that draw on motifs from the Baths of Caracalla. Treatments are designed by Augustinus Bader, and the spa opens daily from 7am to 9pm. The Bvlgari Gymnasivm, developed with Technogym, and a series of guided walks devised around Bulgari's own archives, tracing the jewellery house's Roman references through the city's streets, extend the hotel's offer beyond its own walls.
